PIAA CIPPE Form

The PIAA sports physical form is required for participation in PA sports. This sports physical is valid for one year, June 1, 2022 – May 31, 2023. 

  • In order for the physical to be valid for one year, your daughter’s physician must sign the “PIAA Comprehensive Initial Pre-Participation Physical Evaluation” (page 7), dated after June 1, 2022. 

If your physician has a physical form that their practice uses, please ensure that they also sign the PIAA form. You will be asked to upload only page 7 of the CIPPE to the Family ID registration. Please do not upload without the physician’s signature.

Family ID

All potential fall athletes must complete the information requested on Family ID through the link below by August 1. If you have previously registered for any programs through Family ID, please use the same username and password.

The athletic fee is not paid through Family ID. You will be billed through FACTS once rosters are finalized.

Keep in mind that your daughter will not be permitted to try out without completion of the CIPPE and Family ID. Please do not send your daughter to tryouts without having them completed, as she will be asked to sit and watch. This policy is established in the best interest of the health and well-being of your daughter.
