Athletic Forms

If you did not participate in a fall sport:

The PIAA sports physical form is required for participation in PA sports. This sports physical is valid for one year, June 1, 2021 – May 31, 2022. In order for the physical to be valid for one year, your daughter’s physician must sign the “PIAA Comprehensive Initial Pre-Participation Physical Evaluation” (page 7), dated after June 1, 2021. If your physician has a physical form that their practice uses, please ensure that they also sign the PIAA form. You will be asked to upload this form (Sections 1 through 7) to the Family ID registration. Please do not upload without the physician’s signature.

If you did participate in a fall sport:

Please print out and sign Section 8 of the 2021 CIPPE. You will be asked to upload this form to your winter registration.

Family ID

All potential winter athletes must complete the information requested on Family ID through the link below by November 16th. If you have previously registered for any programs through Family ID, please use the same username and password. You will not have to pay the athletic fee through Family ID. You will be billed through FACTS once rosters are finalized.

Keep in mind that your daughter will NOT be permitted to tryout without completion of the CIPPE and Family ID. Please do not send your daughter to tryouts without having them completed, as she will be asked to sit and watch. This policy is established in the best interest of the health and well-being of your daughter.
