Medical Forms

Important Update: Beginning with the 2024-2025 school year, Villa Maria will be shifting all student medical and athletics information to a new online database, Magnus Health. Information about this new management system and instructions on how to access your Magness Health account were emailed to Villa parents on Friday, July 12th.

Note: When logging in to PowerSchool to access Magness Health, it is best to use Firefox or Google Chrome on a laptop or desktop as the web browser because we have encountered that using the web browser on an iPhone does not show the icon when logged into the parent portal.

The deadline to enter health and athletic (if applicable) information for each child attending Villa Maria was August 1, 2024. It is imperative that all parents who have not yet entered this information do so immediately. Thank you. ***
